At ZAVA, you don’t have to face weight maintenance alone. We can provide you with continued treatment, as well as nutritional and lifestyle advice, allowing you to feel confident and supported for long-term success.
This means we can prescribe weight loss medication even after you’ve reached a healthy weight – a BMI of 25 or less.

What is weight loss maintenance?
The ‘maintenance phase’ of your weight loss journey is when your focus shifts from losing weight to sustaining the amount of weight you have lost and the changes that have helped you achieve your goals.
When entering the maintenance phase, you may want to find ways to continue the changes you have made to your lifestyle (such as your diet, or exercise routine) but at a slower, manageable pace. Alternatively depending on your current health and progress, you might wish to stay on a maintenance dose of your medication, if your doctor approves it.
What treatment options are available in the maintenance phase?
Weight maintenance treatment can involve:
- continuing treatment as it is now
- adjusting your dose
- tapering off your treatment or stopping right away
- continuing your weight loss journey without medication using diet and exercise alone
- restarting treatment down the line – if you regain weight
The maintenance phase of your treatment can involve a number of different steps or choices. The main thing to remember is it’s about finding what works for you to help you keep off the weight that you’ve lost, in the long term.
Why does the maintenance phase matter?
Weight loss is a long-term goal and it’s worth taking the time to make sure the change sticks. Maintenance is the second phase of your weight loss journey and is just as important as the first. It’s about helping you to avoid falling back into old habits to ensure your new healthy lifestyle is here to stay. So, whatever your goal might be, whether you’re tapering down your dose or pushing your progress a bit further, or stopping, we’re here to support you.
How is maintenance different for everyone?
The maintenance phase might be different for different people in a range of ways. If you’re unsure about what to do here are some choices and reasonings based on how patients have felt:

Everyone’s needs are different. What might work for one person in helping them keep the weight off, may not work for someone else. That means there’s no one-size-fits-all approach to weight maintenance – it’s about finding what works for you as an individual.
You may be ready to come off your medication and try out your new lifestyle without support, or you might need a little longer to find the right dose to help you cement your new habits to secure long-term success. The maintenance phase of treatment isn’t the same for everyone. Your goals are unique to you and so are your circumstances.
Is weight loss maintenance with medication right for me?
The choice is up to you. You might want to stay on medication if:
- you’re worried about putting weight back on
- you think you need more time to build your diet and exercise habits
Our doctors can assess your eligibility for maintenance but, ultimately, how you continue your treatment is your choice, you can either: continue your medication and adjust your dose, stay on your current dose, or you may want to taper off or stop.

Can I keep the weight off without continuing weight loss medication?
Yes, in some cases. Sticking to a balanced diet and regular exercise can help you maintain your weight. If you feel confident managing cravings on your own, you may decide to stop your treatment and continue with your new routines.
For some people, stopping medication can make cravings and ‘food noise’ return and weight loss harder to maintain. If this is a concern, you might choose to continue your treatment for longer to give yourself extra support while you adjust to your new routine.

Is it ok to stop treatment right away?
If you’re confident in stopping treatment and maintaining your new lifestyle changes without further support from medication, then that’s fine.
Stopping treatment right away won’t cause withdrawal symptoms. The medication leaves your system slowly over a few weeks so you may notice appetite changes but this should appear fairly gradually.
If you’re sure it’s the right choice for you, you can stop by simply not requesting more treatment after you finish your current dose. Remember you can request to restart treatment after you stop if needed and we will confirm if it’s right for you or not.
When should I consider tapering off treatment?
Tapering off your treatment can be a good choice if you want to work towards lifestyle changes alone without medication, but you want to ‘test the water’ first.
By slowly reducing the dose of your medication, you can experience your appetite returning at a slower rate than stopping straight away. This can help you decide whether you are comfortable coming off treatment entirely.
Can I restart treatment later?
Yes. If you try to maintain your weight by yourself for a while and find you need extra support, you may be able to restart treatment. The only thing to be aware of is that you may need to start from the lowest dose of your treatment again if it’s been more than 2 weeks since your last injection.
What if I want to stop with my medication?
We can support you to do this. You can either stop right away, or slowly lower your dose – ‘tapering off’. There is no need to taper your dose, it’s safe to just stop if you want to. Either way, our doctors can continue to provide advice and you’ll have continued access to resources around managing diet and exercise, and more. We can help you restart down the line if you need to.
What happens if I stop my medication?
If you stop medication you may experience increased cravings, and ‘food noise’, which is when you think about food even when you aren’t physically in need of it. Increased hunger and cravings caused by stopping your medication can make it hard for you to maintain your weight loss. If you continue your treatment with ZAVA, we have a range of resources you can continue to access, including exercise trackers and food diaries.

ZAVA maintenance service allows you to be in control of your weight loss journey no matter what phase you are in, with support so that you don’t have to face it alone, even if you want to come back and restart later. We believe supporting you in keeping the weight off is just as important to us as helping you lose it. Entering the maintenance phase will allow you to continue to receive your medication if you choose, in addition to other resources we provide, helping you sustain the success of your health in the long-term.
How to request maintenance treatment from ZAVA
At ZAVA, weight loss maintenance is simple:
- Request your treatment, just like before
- One of our doctors will review your request, and assess your eligibility
- If approved, your doctor will assess the right dose for you
- Your doctor will monitor your progress on your new dose
Our doctors always review everything as usual, taking into account that you’re now in the maintenance phase of your journey. From there on, your progress will be monitored as you continue with the lifestyle changes you have made up to this point.
How do I know if I am eligible for weight loss maintenance?
You may be eligible for the maintenance phase of weight loss treatment if you:
- have a BMI (body mass index) that meets our criteria for maintenance treatment – this includes after you've reached a healthy weight with a BMI of 25 or lower
- have reached or are nearing your goal weight
- are not actively pursuing further weight loss beyond your goal weight
- have been on the same dose of your weight loss medication for a minimum of 8 to 12 weeks and you’re tolerating it well
- are motivated to maintain progress and open to receiving support
Simply place a request for treatment at the dose you want, and our doctors will review your consultation as usual. You don’t need to tell us you’re in a maintenance phase; that’s normally clear from your assessment and order history.
When can I start weight loss maintenance?
You can start your maintenance phase once you have reached a healthy goal weight, or are close to reaching it, and feel ready to transition to maintaining the weight you have worked so far to achieve.
You may be ready to start maintenance if your:
- weight has stabilised
- diet and exercise plans have become a part of your routine
- motivation has shifted from losing weight to keeping it off
- focus is now on long-term health
You will still need doctor approval to start the maintenance phase, but it’s good to know the signs you may be ready so you can go forward and discuss the next phase with your doctor.
Which dose should I stay on?
Depending on which weight loss injection you have been prescribed for your weight loss journey, each has different strengths which you can use as your maintenance dose depending on your health, needs, and progress.
Why it’s important to think about your dose during maintenance
The maintenance phase can last longer than the weight loss phase of your treatment, so it’s important that you find a maintenance dose that works for you. Our doctors can work with you so you can be secure in your new weight long-term.
Your doctor can suggest a plan that works for you, both in terms of the right level of appetite control and minimising the risk of any side effects during your weight loss journey.
Why might your dose be different during the maintenance phase
When it comes to maintenance treatment, you can help decide which dose is right for you. This dose can be different for different people and potentially, any dose can be clinically effective for maintaining your weight, it just depends on you and your needs.
How to decide which dose you need
Questions to ask yourself when figuring out the right dose for you for maintaining your weight:
- Have I had side effects that have been difficult to manage?
- Do I feel my current dose is right for maintaining my current weight?
- How much exercise am I committed to doing?
- What is my awareness of my diet and food habits?
- What is my budget for treatment?
Staying on your current dose
If you find that you have been tolerating your current dose well and want to stay in control of your cravings, keeping on your current dose is a good option. For example, you might feel you need extra support with keeping the weight off as you adjust to your new diet and exercise routines. However, if you find that you’re continuing to lose weight beyond your goal, you may want to rethink your dose.
Dropping your dose
If you find that you have been tolerating your current dose well and want to stay in control of your cravings, keeping on your current dose is a good option. For example, you might feel you need extra support with keeping the weight off as you adjust to your new diet and exercise routines. However, if you find that you’re continuing to lose weight beyond your goal, you may want to rethink your dose.
What if I have already stepped down a dose and I am still experiencing side effects?
Speak to your doctor, as the treatment you are currently taking may not be suitable for you.
Increasing your dose
If you have been feeling as though your cravings are returning and in turn less control of your weight compared to when you first started treatment, this could be a sign to enter the maintenance phase on a dose increase. Factors that can cause your body to become less responsive to your treatment include:
- hormonal imbalances (often caused by stress)
- lifestyle changes
- inconsistent diet
- lack of exercise routine
- underlying health changes
Weight loss medication works differently on everyone. Some people simply find that their body becomes more tolerant of the medication, and as a result they need to increase their dose. If you are at the maximum maintenance dosage, this could be a sign that you need to try alternative treatment approaches.
Your doctor will be able to assess your needs so you can move forward with your long-term plan for success.
How long can I stay on maintenance for?
You can stay on your maintenance treatment for as long as it is safe for you to do so. This means as long as our doctors can still safely prescribe your treatment to you, they will.
Everyone’s journey with weight loss is unique to them. So, for however long you feel that you need extra support with keeping the weight off, we can provide that support. If at any point your circumstances change and our doctors can no longer prescribe treatment safely to you, they will let you know next time you request treatment.
What if I regain weight after stopping medication?
If you stop treatment and you regain weight, you may be able to restart your treatment. You can request treatment again, the same way you did before. Just like when you started treatment, one of our doctors can check whether it is safe for you to restart your treatment. A doctor will consider:
- your current BMI – this does not necessarily need to be as high as the requirements for starting treatment in the first place
- your current lifestyle
- the length of time since you stopped treatment
- how well you tolerated your treatment before stopping
- your treatment and health history
We know that maintaining weight can be just as challenging as losing it. Life can get in the way, and sometimes progress isn’t always consistent. Staying committed to lifestyle changes such as exercise routines and diet can be difficult without the help of weight loss medication.

He completed the Royal College of GPs membership exam in 2007. He's worked within the NHS as well as in the private sector, helping him gain unique insights into the future of healthcare.
