How does the Wegovy pill work?

If you’re already aware of how weight loss injections work, the Wegovy pill works in a similar way. It reduces appetite making it easier to eat fewer calories and lose weight. The main difference is that, because it’s absorbed through the digestive system and not under the skin, the pill is slightly less effective.

The Wegovy pill contains the same active ingredient as Wegovy injections, so it works the same way. It helps support weight loss by:

  • lowering your appetite, meaning you do not feel as hungry
  • reducing ‘food noise’, so you do not feel like you need to think about food as often
  • slows digestion, which means food stays in your stomach and you feel fuller for longer

More about how the Wegovy pill works

The active ingredient in the Wegovy pill, semaglutide, belongs to a group of medications called glucagon-like peptide-1 (GLP-1) receptor agonists. It works by mimicking the hormone GLP-1, which is responsible for controlling appetite, digestion, and blood sugar.

The semaglutide in the Wegovy pill reduces your appetite and slows down your digestive system. This makes following a diet and exercise plan easier, allowing you to eat fewer calories without feeling as hungry.

Because the Wegovy pill is absorbed after swallowing it, it is not absorbed like the injections, which go directly under the skin. As a result, the Wegovy pill has the same effect as injections, but it leads to slightly less average weight loss.

17% is the average amount of starting weight people lose, but that is not the case for everyone. Still, most people will lose at least 5% of their starting weight. Results from clinical trials on the Wegovy pill show that:

  • 79% of people lose at least 5% of their starting weight
  • 63% of people lose at least 10% of their starting weight
  • 50% of people lose at least 15% of their starting weight
  • 30% of people lose at least 20% of their starting weight

When you can expect to see weight loss

Everyone’s weight loss journey is different, and it can take time to see significant weight loss during treatment. Still, you should notice a drop in your appetite fairly early on, and if you stick to your diet and exercise plan, you’re likely to see your weight go down over time.

Month 1

After 4 weeks of treatment, people in clinical trials lost an average of 2% of their weight. You might notice some appetite suppression, but the lowest dose is more for getting your body used to the Wegovy pill to lower the chances of side effects. It’s common to get side effects in the first few weeks, which should settle over time.

Month 2

By week 8 (around 2 months) after starting treatment, people lose an average 4% of their starting weight. You’ll be on the 4mg dose, which should come with stronger appetite-suppression effects, and you should be gradually finding it easier to stick to your reduced-calorie diet. Side effects may come back or happen for the first time with a dose increase, but should be mild and settle within a few weeks.

Month 3

By week 12, which is just under 3 months in, people lose an average 6% of their starting weight. Sticking to your diet and exercise plan will make sure your weight loss stays consistent. As month 3 comes with another dose increase to 9mg, you may notice some very common side effects that get better within a few days to a few weeks.

Month 4

By week 16, just under 4 months in, people lose around 8% of their weight. This means that each month, you could be losing around 2% of your body weight, or more in some cases. At the start of month 4, you’ll start the highest dose, 25mg, which is also the maintenance dose.

Side effects may return, but should settle once your body adjusts. There’ll be no more dose increases, and you can stay on 25mg for as long as you need to lose your goal weight, and then for maintenance if you want to keep using weight loss medication to maintain your weight.

Month 6

By week 24, people in clinical trials lost an average of around 10% of their starting weight. As you may have been on the 25mg maintenance dose for a while, your body should be fully adjusted to the medication, and any side effects should have settled for most people.

By this stage, healthy eating habits and regular exercise can start to feel more routine. While weight loss may not happen at the same pace every month, continuing to stick to your plan gives you the best chance of reaching your goal weight.

Month 15

By around 64 weeks into treatment, people in clinical trials lost around an average of 17% of their starting weight. If you've reached your goal weight, or you’re getting close, you may be ready to enter the maintenance phase. This is where the focus shifts from losing weight to helping keep up your success.

Along with continuing with your diet and exercise plan, you can continue to use medication at a maintenance dose. This dose may be lower than the dose you’ve used to lose weight up to this point. Our doctors can help you choose the right maintenance treatment option for you.

Who can take the Wegovy pill?

The Wegovy pill may be right for:
  • people who have a BMI of 30 or more
  • people who have a BMI of 27 or more and a weight-related health condition, like high blood pressure
  • people who are already receiving weight loss treatment and want to switch

If you meet the body mass index (BMI) requirements for Wegovy pill treatment, it could be a good choice for you. As long as you meet the criteria and there are no safety reasons why you cannot use the Wegovy pill, it should be right for you.

Why is BMI verification important?

The Wegovy pill is only meant as a weight loss treatment for people who are obese or overweight with certain other risk factors. We can only safely and legally prescribe Wegovy online if we check to make sure people meet the prescribing criteria.

This includes checking that people are the BMI they say they are. As well as asking people to tell us their BMI during their treatment consultation, we also ask them to take pictures so we can confirm their BMI and prescribe treatment safely.

Is there anyone who can’t use the Wegovy pill?

Wegovy pills are not suitable if you:

  • have an allergy to semaglutide or the other ingredients in the tablets
  • are planning a pregnancy (semaglutide should be stopped 2 months before trying for a baby)
  • are pregnant or breastfeeding

You should also speak to your doctor before starting the Wegovy pill if you:

  • have kidney or liver problems
  • are diabetic and have diabetic eye problems
  • have had pancreatitis or acute pancreatitis
  • have severe heart failure
  • have severe gastrointestinal disease, including gastroparesis
  • have been diagnosed with non-arteritic anterior ischaemic optic neuropathy (NAION)
  • have had allergic reactions to other GLP-1 receptor agonists
  • are using oral contraception – side effects like vomiting and diarrhoea can reduce the absorption of the oral contraceptive pill and make it less effective

If you have a chance of getting pregnant and are sexually active, you must use contraception while taking Wegovy pills, as the effect of semaglutide on an unborn baby has not been tested. If you get side effects that stop your oral contraceptive pill from being effective, or you miss a pill, use condoms or speak to your doctor or a sexual health clinic about the coil or implant.

Switching from injections to the Wegovy pill

If you’re already using weight loss injections, you may be wondering what the process is if you’d like to switch to the new Wegovy pill. Switching is possible if a doctor agrees, and it’s possible to move onto a higher dose of Wegovy than the starter dose.

As long as injections are still right for you, you should be able to make the switch to the Wegovy pill:

  • A doctor will need to approve the switch.
  • You may be able to start on a dose higher than the Wegovy pill starting dose, depending on your current injection dose.
  • If it’s been more than 2 weeks since your last injection, you’ll need to begin on the Wegovy pill starting dose.

Please note: The Wegovy pill offers a needle-free alternative for weight loss treatment, but weight loss injections are still the most effective treatments available. If you’re currently on weight loss injection treatment and you’re happy with it, there’s no need to switch.

Why do people consider switching?

There are a number of reasons why you might want to switch to pills over weight loss injections, including:

  • being worried about injecting weekly
  • having a preference for taking medication daily or find it easier to keep track of this way
  • travelling often – injections must meet special storage requirements (in the fridge until opened) and require you to bring spare needles and somewhere to safely dispose of them, such as a small sharps bin
  • having anxiety or phobias about needles
  • getting strong reactions or side effects around the site of injection

Switching from Wegovy injections to pills may be possible if a doctor agrees it’s still right for you. Since they contain the same active ingredient, it’s likely you will react similarly. The main difference will be the way the medication is absorbed. Wegovy injections are absorbed under the skin and the pills are absorbed in the stomach, so you may get a different reaction from your digestive system in response to the pills.

The dose of Wegovy pill you can switch to depends on your current Wegovy injection dose. If you’re on 1.7mg Wegovy injections or higher, you can switch to the highest Wegovy pill dose.

Switching from Mounjaro might feel like a more significant transition because tirzepatide is a different active ingredient, even though both are GLP-1 receptor agonists. Because tirzepatide works by mimicking 2 gut hormones instead of 1, it’s often considered to have a more enhanced effect on suppressing your appetite and keeping you feeling full. You may also have a different response to side effects on Wegovy pills, even if you did not have the same side effects on Mounjaro.

If you’re taking 7.5mg or higher of Mounjaro injections, you can usually switch to the maximum maintenance dose of the Wegovy pill.

How to take the Wegovy pill

If you’re looking for a less complicated way to take weight loss medication, the Wegovy pill could help. Taking the pill is as simple as most other oral tablets. However, it does need to be taken at the right time, without food and with only a small amount of water.

The Wegovy pill is a daily treatment that you swallow whole with a small amount of water (no more than 120ml) when you first wake up. It needs to be taken on an empty stomach and you should wait 30 mins before eating or drinking anything else.

Step-by-step guide for taking your Wegovy pill

The Wegovy pill needs time to absorb in your stomach to work properly. You must follow the instructions carefully so that it’s effective. To take the Wegovy pill, follow these steps:

  1. Take the pill when you first wake up, as it must be taken on an empty stomach after at least 8 hours of fasting (this includes water).
  2. Swallow 1 Wegovy pill whole with a full sip of water (up to 120ml). Do not have more than 120ml of water, as this could affect the pill's absorption. The pill must not be chewed, split, or crushed.
  3. Wait 30 minutes after taking it before having anything else to eat or drink, or taking any other oral medications.

Important: If you take other pills when you wake up, they should not be taken alongside the Wegovy pill. Wait at least 30 minutes before taking them. Ask your doctor for advice if you are unsure.

Tips for using your treatment

How to remember to take your pill

In order to get the most effective treatment possible, it’s important you remember to take your pill every day. There are few ways you can improve your chance of remembering, including:

  • using a pill box
  • setting an alarm on your phone
  • using a pill reminder app
  • keep your pill by your bed
  • adapt taking your pill into your routine when you wake up

What to do if you forget

If you forget to take your Wegovy pill, take it as soon as you remember if:

  • it’s still around the same time you’d usually take (give or take around 1 hour)
  • you still have not eaten, drunk, or taken any other medications by mouth

If you forget and no longer have an empty stomach, skip the missed pill and continue with your schedule the next day. If you miss more than 7 pills in a row, you’ll need to restart at the lowest dosage.

Wegovy pill dosage

Like Wegovy injections, the Wegovy pill has a dosage schedule where you start on a low dose and increase over time to help your body get used to the medication.

Dose of wegovy pill 1.5mg 4mg 9mg 25mg

There are 4 doses of the Wegovy pill available. You will start on the lowest dose of 1.5mg with the option to increase your dose after at least 1 month on your current dose. Although, you don't have to move up if your current dose is working well for you. The doses are:

  • Month 1 – 1.5mg
  • Month 2 – 4mg
  • Month 3 – 9mg
  • Month 4+ – 25mg

How to manage your dose

You’ll normally follow the dosage schedule, so you'd increase your Wegovy pill dose once a month. The packets last 30 days at each dose, so in some cases, this could run slightly longer or shorter than a month. You must finish your 30 pills before starting the next dose. If you want to follow the dosage schedule, you’ll start the highest dose at month 4.

While you cannot increase your dose early, you can stay for longer at a lower dose if you are:

  • getting side effects that you want to settle before starting the next dose – your doctor may also advise this
  • seeing consistent weight loss so far, and want to see how things go before increasing again

If you're not sure whether to increase your dose, or if you think you're having side effects, you can message your doctor for free at any time through your online account. It’s very important that you let them know about any side effects that do not get better after a few weeks, get worse, or that are affecting your day-to-day activities.

Wegovy can cause serious side effects in some cases. See a doctor right away if you experience complications of diabetic eye disease, vision changes, inflamed pancreas, kidney or bladder stones, or a severe allergic reaction.

Other side effects

Common side effects of Wegovy pills (semaglutide) include:

  • dizziness
  • burping
  • gas (flatulence)
  • bloating
  • inflammation of the stomach lining (gastritis)
  • reflux or heartburn (gastro-oesophageal reflux disease (GERD))
  • taste changes
  • gallstones
  • hair loss
  • taste changes
  • abnormal skin sensations, like burning, stinging, or pins and needles (dysaesthesia)
  • low blood sugar (hypoglycaemia) in people with type 2 diabetes

Uncommon side effects of Wegovy pills (affecting up to 1 in 100 people) are:

  • low blood pressure
  • feeling dizzy or lightheaded when standing after sitting or lying down because of a sudden drop in blood pressure
  • fast heartbeat
  • increased pancreatic enzyme levels in blood test results
  • delayed stomach emptying
  • low blood sugar (hypoglycaemia) in people who do not have type 2 diabetes
  • increased levels of bilirubin in your blood, signs include yellowing of the skin and whites of the eyes (jaundice)

Wegovy pill FAQs

Is the Wegovy pill available on the NHS?

Not right now. Although it is licensed and approved by the MHRA to be prescribed for weight loss in the UK via private healthcare providers like ZAVA, it is still undergoing review for availability on the NHS.

All other approved weight loss treatments are available on the NHS, including Wegovy, Mounjaro, Nevolat, and orlistat. Official guidelines have not been released yet, including when oral Wegovy will be available on the NHS or how you will be able to access it.

GPs can prescribe Mounjaro (under very special guidelines for those most at-risk of weight-related medical problems) or orlistat, but other treatments are accessed with a referral to a specialist weight management service.

Is oral semaglutide the same as Wegovy?

Yes, oral semaglutide is the same as Wegovy. Wegovy pills are an oral tablet that contains semaglutide. There is also another oral semaglutide pill called Rybelsus, but this is for type 2 diabetes.

Is the Wegovy pill safe?

Yes, the Wegovy tablet is safe as long as it’s been prescribed to you and you take it correctly. There is a chance of side effects, as with any medication, and you should report these to your doctor so they can monitor your progress and make sure it remains safe. There are also benefits of taking Wegovy, including improved cholesterol levels and heart health.

What if I’m not losing weight taking the Wegovy pill?

If you’re not losing weight while taking the Wegovy pill, there could be a few reasons, such as:

  • you’ve not been taking it correctly – Wegovy tablet must be taken as prescribed by your doctor
  • you’ve not given it enough time – the best results are seen after 64 weeks or more
  • you’re not in a calorie deficit – the Wegovy pill helps you lose weight while following a diet and exercise plan, including a calorie deficit for weight loss
  • you need to reach the maintenance dose – the 25mg dose has been proven to give the highest results
  • you’re in a weight loss plateau

The Wegovy pill has been proven to help 79.2% of people lose at least 5% of their weight. If you’re following the dosage schedule and an appropriate diet and exercise plan and still not seeing any weight loss, speak to your doctor. There’s a chance Wegovy is not working for you.
